To obtain a REAL ID in Pennsylvania, you must provide the following documents: 1. Proof of Identity and Lawful Status (choose one):

  • Certificate of U.S. Citizenship (Form N-560 or N-561)

  • Valid, unexpired Permanent Resident Card (Green Card) (Form I-551)
  • Unexpired foreign passport with valid U.S. visa and I-94 form showing permanent residence

2. Proof of Social Security Number (must show full 9 digits and current legal name):

  • Social Security card
  • W-2 form
  • SSA-1099 form
  • Non-SSA-1099 form
  • Pay stub

3. Two Proofs of Pennsylvania Residency:
Examples include:

  • Current, unexpired PA driver’s license or photo ID card
  • PA vehicle registration card
  • Auto insurance card
  • Utility bill (cellphone, cable, electric, gas) with your name and address
  • Post-marked mail/package labels
  • W-2 form or pay stub
  • Lease agreements or mortgage documents

4. Proof of Legal Name Change (if applicable):

  • Certified marriage certificate
  • Court order approving name change
  • Amended birth certificate showing name change
    (Note: If your passport reflects your current name, no additional proof of name change is needed.)
